When travelers plan a trip to Peru, they usually follow this order:
Arrive in Lima → Fly to Cusco → Visit other cities like Iquitos, Arequipa or Puno afterwards.
While this seems logical, it actually causes many avoidable problems, mostly related to Cusco’s extreme altitude.
As a tour operator, I see this every month: travelers losing tours, flights, and prepaid experiences because they got sick in Cusco before visiting the rest of Peru.
Here’s why it’s much safer to visit Cusco at the end of your trip, and how to plan your itinerary properly.
What is altitude sickness and why does it hit so hard?
Cusco sits at 3,399 meters (11,152 ft).
Most travelers are not used to this elevation, and symptoms often appear:
-
headaches
-
extreme fatigue
-
nausea
-
loss of appetite
-
shortness of breath
-
difficulty sleeping
Some people adapt quickly, others are completely incapacitated for 1–3 days.
The common mistake: going to Cusco first
Many travelers think:
“I’ll arrive, rest one day and I’ll be fine.”
Reality check:
✅ Acclimatization takes time
✅ Symptoms can appear later
✅ Everyone reacts differently
✅ If you get sick before another tour or flight, you may lose it
This is a major problem when travelers visit destinations like:
-
Iquitos & the Amazon
-
Northern Peru
-
The coast
-
Other jungle or low-altitude regions
Real consequences (from a tour operator’s experience)
I’ve seen travelers lose:
-
flights to Iquitos
-
prepaid jungle tours
-
non-refundable reservations
-
entire travel days stuck in bed
All because they visited Cusco first
The ideal order to visit Peru is to begin with low-altitude destinations—such as Lima, Iquitos, Paracas, the Amazon, Trujillo, Chiclayo or Tarapoto—and save Cusco as the final stop of your trip,
since its extreme altitude (3,399 m / 11,152 ft) often causes discomfort for many travelers, including headaches, fatigue, nausea or insomnia, which can disrupt plans and lead to missed flights,
tours or non-refundable bookings. Starting with coastal or jungle regions allows you to enjoy Peru safely and with full energy, while visiting Cusco at the end greatly reduces the risk of
altitude sickness and gives your body time to adapt naturally; and if symptoms do appear, you won’t have other important destinations left to lose. For this reason, the best approach is to
explore the Peruvian jungle—such as Iquitos—and the coast first, and then finish your journey with Cusco, Machu Picchu and the Sacred Valley, ensuring a smoother, safer and more enjoyable
experience.
The Smartest Peru Itinerary: Suggested Routes and Altitude Tips
A smart way to plan your trip through Peru is to follow an itinerary that starts in low-altitude regions and leaves Cusco for the end, such as spending 7 days between Lima, Iquitos and then Cusco, or a 10-day plan beginning in Lima, taking a 3–4 day Amazon jungle tour in Iquitos, and finishing with Machu Picchu and the Sacred Valley; this order not only makes geographical sense, it also greatly reduces the risk of altitude sickness by giving your body more time to adapt. To further minimize any discomfort once you reach Cusco, it’s best to stay hydrated, avoid alcohol on the first day, eat light meals, move slowly, rest the first few hours, try coca tea or coca candies, and skip strenuous tours right upon arrival; by combining these tips with a well-thought-out itinerary, you’ll enjoy the jungle and the Andes without risking missed flights, canceled tours or losing valuable travel days.
